Serious, life-threatening, or fatal injuries: There are reporting requirements established by the Industrial Commission of Arizona, especially in the case of fatalities; therefore, it is imperative that serious, life-threatening, or fatal injuries must be called in to the Industrial Commission of Arizona at 602-542-5795 or toll-free at 855-268-5251. The State of Arizona-Risk Management must also be immediately notified at 602-542-2182 or via email at [email protected] and [email protected]. Any fatality must be reported immediately to the state OSHA Office and Risk Management.

LOSS PREVENTION
Risk Management provides Loss Prevention Consultants, Industrial Hygienists, and Environmental Specialists to assist agencies in developing and implementing loss prevention programs. The consultant can also provide assistance in solving everyday loss prevention problems.

PROPERTY AND LIABILITY
The Property & Liability Claims Section investigates, evaluates, negotiates and settles claims for: State property, State vehicles, Liability claims.
For claim information or to request a loss report forms for auto accidents, property losses and liability claims, contact the Claims Section at: (602) 542-2180.
Reporting Liability Claims: All losses must be reported as soon as possible to Risk Management at (602) 542-2180. Agencies must report all liability claims within 1 day if there is bodily injury, death, or if damage is expected to exceed $10,000. All other liability claims are to be reported within 10 days.
Reporting Property Claims: All losses must be reported to Risk Management at (602) 542-2180 in Phoenix or at (520) 628-6700 in Tucson. Agencies must report significant property damages within 1 day if damages are expected to exceed $10,000. All other losses must be reported as soon as possible, but no longer than 10 days of the date of discovery. Property losses not reported to Risk Management within 90 days of date of discovery will not be covered. (See Administrative Code R2-10-104
